CommunityHealth, the country’s largest volunteer-based free clinic, is opening a new location in Little Village and seeking volunteer primary care providers to join the team.
CommunityHealth at Enlace, located at 2759 S Harding Avenue in Little Village, is a clinic situated within an established social service agency (Enlace Chicago), allowing us to bring care closer to home while more closely collaborating with local services. Patients can access provider visits (onsite and telehealth), lab visits, medication pickup, and health education classes, along with the many social services offered by Enlace.
We are seeking primary care providers willing to serve at least 1x/month on a Wednesday evening or Saturday morning, either onsite or via telehealth. The telehealth experience is greatly enhanced for both provider and patient, as the patient will be onsite with an MA facilitating. Telehealth patients will have fresh vitals taken and have access to point of care labs.
